Required contents of notice (17 U.S.C. § 512(c)(3))
1. A physical or electronic signature of the copyright owner or authorized agent.
2. Identification of the copyrighted work claimed to be infringed.
3. Identification of the material claimed to be infringing and its location (URL).
4. Your contact information: address, telephone, email.
5. A statement of good-faith belief that the use is not authorized.
6. A statement, under penalty of perjury, that the information is accurate and that you are the copyright owner or authorized to act on the owner's behalf.
Counter-notices
If you believe material was removed by mistake, you may submit a counter-notice containing the elements set out in 17 U.S.C. § 512(g)(3) to the same agent.
Repeat infringers
We will terminate the accounts of users who are found to be repeat infringers.
